Please email me the confirmation number prior to the medical, when I will use it to take over your electronic record and add the medical data.<\/p>\n<p style=\"font-weight: 400;\">Please make sure you fill all dates in <strong>U.S. format<\/strong> and enter your name and address exactly as you want it to appear on your medical certificate.
